Documents Required to Complete Registration
PRINT THIS FORM AND BRING IT WITH COPIES OF THE FOLLOWING DOCUMENTS STATED BELOW.

DOCUMENTS REQUIRED TO COMPLETE REGISTRATION
1 Recent Passport Sized Photo
A Copy of :
1. Student’s Birth Certificate
2. Student Passport if student was born outside of Trinidad and Tobago
3. Student’s Immigration Permit (if applicable)
4. Father’s form of Identification/Mother’s form of Identification /Guardian’s form of
Identification (a minimum of 1 form of Identification needed)
5. Proof of Address for student (Utility Bill with Parent’s /Guardian’s Name)
If utility bill is not in parent/legal guardian’s name, the following is needed:
- a letter of authorization from the person whose name is on the utility bill
- a copy of ID Card of the person whose name is on the utility bill
6. If you are the legal guardian of a student, YOU MUST BRING THE ORIGINAL AND
COPY OF ANY LEGAL DOCUMENTS PROVIDED BY THE COURT.
7. School Immunization/vaccination card
